WHO CAN REFER
We welcome referrals from professionals and organizations that support individuals experiencing housing transitions, including:
- Social workers and case managers
- Emergency and transitional shelters
- Re-entry and justice-involved support programs
- Veteran service organizations
- Community-based nonprofits
- Faith-based and outreach organizations

REFERRAL PROCESS
Step by Step Referral Process
Step 1: Submit a Referral
Referring partners complete the referral form and provide relevant background information.
Step 2: Program Review
Each referral is reviewed to determine program fit, availability, and alignment with participant needs.
Step 3: Participant Intake
If approved, the participant completes the intake process and reviews program guidelines and expectations.
Step 4: Program Placement
